COMPLAINT FORM

This complaint form is for complaints about pharmacy non-compliance with California’s pharmacy laws, including SB 362 banning quotas placed on individual pharmacists and SB 1442, the “No Pharmacist Left Alone” law. We need your help to make sure the laws we passed are enforced and pharmacists can do their job taking care of their patients. 

This form will be forwarded to an attorney for the United Food and Commercial Workers (UFCW) and this form will remain confidential under the attorney-client privilege. The attorney will analyze the complaint in light of the law, make a determination if a complaint reveals a possible legal violation, and then lodge a complaint to the Board of Pharmacy on your behalf.
